This letter is to inform the Alabama Department of Environmental Management (ADEM)
that Acbieva, Inc. has elected PPM Consultants, Inc. (PPM) as their Trust Fund contractor
for completion of investigative and corrective actions at the referenced site. A copy of the
signed contract between Achieva, Inc. and PPM (Authorization to Proceed) is enclosed.
Please copy PPM on all correspondence for the referenced incident.

PROJECT DESCRIPTION
Client Name: Achieva,Inc. ATTF Incident No.: UST21-12-04
Project Name: Former Quick Serve No. 25-Investigative&Corrective Actions per ADEM Requirements
Project Location: 2909 East South Boulevard,Montgomery,Alabama
Services to be Provided: Perform consulting services in accordance with ADEM guidelines and reasonable rate
scenarios established by the Alabama Tank Trust Fund (ATTF), per the scope of work and costs described in the
individual ADEM Cost Proposals to be prepared and approved prior to each phase of work. Signature of this
authorization to proceed indicates the Client's agreement to allow PPM Consultants, Inc. to serve as their ATTF
contractor for the duration of the project or until this contract is terminated at the request of Client or PPM. In the
event of termination or failure by Client to execute ATTF Payment Requests, the Client hereby agrees to pay
PPM for costs incurred for partially completed cost proposals, work plans, and/or project work requested by
ADEM, but not paid by the ATTF. Client agrees to sign payment requests for all completed project work. Per
the attached Business Terms and Conditions, PPM will direct bill and accept payment directly from the ATTF.
Client agrees es to process all paperwork within five business days of receipt.
PROJECT TERMS
Start Date: Immediately upon receipt of Authorization to Proceed
Project Fee: Not to exceed the ATTF approved costs as approved in each individual ADEM Cost Proposal.
Terms: Per attached Business Terms and Conditions, except PPM will direct bill and accept payment directly_
from the ATTF. Client agrees es to process all required paperwork within five business days of receipt from PPM.
