Permit Verification Checklist
There did not appear to be any recent construction at the site. A large portion of the site appeared to be well vegetated and
stable. However, large bare areas remain at the site. No significant erosion or sedimentation was noted at the site.

Operations & Maintenance Checklist
1.01. Was a rain gauge observed onsite?
Photo Taken:10/4/2019 10:44:35 AM
A rain gauge was employed at the site, however, the rain gauge contained water and had not apparently been checked for some
time.
Recommendation:
BMPs have been implemented at the site. Silt fencing has been installed in some sections and well vegetated buffers exist at
the site. There are, however, large bare areas where construction has not taken place for thirteen days or longer. Bare areas
should have temporary seed, mulch, and/or other means of stabilization where construction has not taken place for thirteen
days or longer. No significant erosion or sedimentation associated with the bare areas was noted.
2.01. Are BMPs properly implemented and maintained?
