International Paper - ALD005557004
According to information received by this Division on June 20, 2001 the information
on file for International Paper, ALD005557004, had changed. The facility that is in
operation at 100 Jensen Road in Prattville has a new contact.
Any change of ownership or facility information must be made using ADEM Form
8700-12, Notification of Regulated Waste Activity, in order for an EPA ID number to
be correct.
Enclosed is the most current ADEM Form 8700-12. Please complete this form and
return it to this Department promptly. Departmental records and EPA ID number for
this facility will be updated within 10 days of the Department's receipt of a correctly
completed form and certification fee.
Note: Effective April 13, 2001, facilities subject to regulation under ADEM
Administrative Code 335-14 - Hazardous Waste Program Regulations are required to
notify the Alabama Department of Environmental Management (ADEM) of waste
activity on an annual basis.

Eddie Pope,Mill Manager of the International Paper Prattville Mill,retired,effective May 31,
2001.The new Mill Manager,Roman Gallo, is immediately responsible for operations at the
facility. Attached is a letter from Dennis J.Colley,Vice President,Containerboard and Kraft
Papers,granting the site manager signatory authority for submission of data and reports for the
Prattville Mill.

the name and on behalf of International Paper all environmental reports, documents,
plans or otheiinformation related'to the Prattville Mill require by federal, state, or local
